Rockall Score

Two different low-risk thresholds. Before endoscopy only a clinical score of 0 counts as low risk; after endoscopy the threshold is a full score of 2 or less. A patient can clear the second without clearing the first.

Rockall and Glasgow-Blatchford answer different questions. Blatchford predicts need for intervention and decides who can go home before endoscopy. Rockall predicts rebleeding and death, and its full form needs the scope. They are complementary, not alternatives.

Comorbidity jumps 0 to 2 to 3, with no 1-point tier, and stigmata of recent haemorrhage jumps 0 to 2. Both gaps are as published, not omissions here.

Shock is judged on blood pressure first: a systolic below 100 scores 2 whatever the pulse is doing, including a normal one — worth remembering in a patient on beta blockers, who may not mount a tachycardia at all.
