Record the components, not just the total.E3 V4 M5 carries information that "12" does not, and the motor score is the component that tracks deterioration most usefully.
Intubated patients:the verbal response can't be assessed. The usual convention is to record it as 1T (or the whole score as, say, E4 VT M6) rather than scoring a genuine 1 — this calculator has no separate "not testable" option, so treat a verbal score of 1 in a ventilated patient with that in mind.
Mild 13–15, moderate 9–12, severe 3–8 is the conventional head-injury classification rather than part of the original scale.
